Event & Meeting Registration Form

In order to minimize scheduling conflicts, this form is required to be submitted and approved by the Board/Executive Committee or Minister in order to reserve space at Unity Church for any event, including meetings. It also is required for church community events such as the picnic even if held elsewhere. Confirmation will be sent within 7 (seven) business days to the appropriate person(s).

Please complete the form completly. We will make every effort to include related information in the monthly calendar, newsletter and bulletin reminders.

These will be printed and filed by month in a Facility Use notebook in the main office.
